Requirements:

  1. Coaching session 2

  2. Complete CE

  3. Complete 40 tests

  4. Submit treatment guide and home care sheet

SILVER LEVEL

Test – You’re Testing, Start Treating

Goal: Master how to use the Treatment Guide

GOLD LEVEL

Clear – You’re Treating, Start Clearing

Goal: Master post testing for clinical end point

Requirements:

  1. Coaching session 3

  2. Complete CE

  3. Complete 60 tests (30 of which have to be post tests)

  4. Complete 2 case reviews: Present each case in 50 words or less including:

    1. Treatment guide for pre and post test when goal was achieved

    2. Treatment guide for pre and post test when goal wasn’t achieved

  5. Quiz​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​

PLATINUM LEVEL

Collaboration – You’re Clearing, Start Collaborating!

Goal: Master clinician guidebook and resources

Requirements:

  1. Coaching session 4

  2. Complete video series

  3. Complete 100 tests (40 of which have to be post tests)

  4. Submit video of presenting guidebook:

    1. Cover

    2. Levels of periodontal disease

    3. Presentation guide

    4. Treatment guide

    5. Homecare sheet

    6. Collaboration form

  5. Quiz

Provider Receives: 

  1. Platinum seal for certificate of recognition

  2. Platinum “certified” patch

  3. Website listing

BRONZE LEVEL

Requirements:

  1. Complete Coaching Session 1

  2. Complete 40 tests

Office Receives: 

  1. Website listing

  2. Bronze seal with Certificate of recognition

  3. Bronze certified seal to use on their website and marketing materials

SILVER LEVEL

Requirements:

  1. Complete Coaching Session 2

  2. Complete 80 tests

Office Receives:

GOLD LEVEL

Requirements:

  1. Complete Coaching Session 3

  2. Complete 150 tests (50 must be post tests)

PLATINUM LEVEL

Requirements:

  2. Complete 250 tests (100 must be post tests)

Step 1: Enroll in the Certification Process

Step 3: Complete minimum of 35 tests

1. Test patients for subclinical disease, no clinical signs of bleeding or bone loss

2. Test patients with gingivitis, just have inflammation & bleeding

3. Test patients with clinical periodontal disease, bleeding & bone loss

Step 4: 

Clear Aa and Pg (Undetected) + Td, Tf, and Fn to minimum of Green Level on 3 tests

1. Submit initial test

2. Submit treatment guide

3. Submit post test

4. Submit Post-testing Protocol
